Workers Compensation for contruction workersAt the Spine Center, our physicians emphasize a comprehensive treatment of Spinal work-related injuries. We are devoted to the diagnosis and treatment of all conditions related to the spine. We treat acute injuries and work emergencies, chronic and repetitive stress injuries, and perform Independent Medical Evaluations (IMEs) as well as offering second opinion examinations for even the most complex of cases. The physicians of the Spine Center have been the leading providers of orthopedic spine services for injured City of Chicago workers for many years. We frequently treat police officers and firefighters injured in the line of duty and take great pride in our excellent record of restoring injured city workers to full health and function. We provide the same level of excellence in treatment of all injured workers which has resulted in outstanding outcomes.

The Spine Center strives to ensure expeditious scheduling of appointments for injured workers, typically with 24 hours. Our physician forms and reports are available within 5 business days, with quick reports available the day of service. We will complete any employer work status at the time of service.

Any necessary diagnostic tests will be arranged by our office staff, and we will take care of any required W/C authorization procedures. We work closely with nurse case managers and will provide any necessary documentation to the injured worker at the time of the visit.

Appointment Scheduling

To schedule an appointment with our physicians for a work-related injury, simply call our office location listed on this website and inform the appointment/reception desk that you need to be evaluated for a work-related injury.

Please bring the following completed form to your appointment:

  • New Patient Form
  • Workers Comp Information Form and/or
  • Auto Accident Information Form

IME/Second Opinion Scheduling

To schedule and Independent Medical Evaluation or a second opinion for a patient, please call our office at (847) 698-9330, option 1.

Independent Medical Evaluation (IMEs) are often required to answer specific medical questions, including relationships of diagnosis to specific injury, accident or illness, further treatment recommendations, extent of permanent impairment or disability, and other information as needed. Our practicing physician consultants possess a broad range of credentials, and board certifications. We make every effort to schedule and complete the IME as soon as possible. After completion, we process and send a comprehensive report within 5 business days.
